WAIVER OF LIABILITY AND RELEASE FORM FOR THE BODHI CLUB

Introduction / Acknowledgment:

I declare that I am over 18 years of age (or have otherwise provided parental consent) and acknowledge and understand that I have voluntarily chosen to participate in the classes and activities offered by The Bodhi Club.

By signing below, I acknowledge that I have read, understood, and voluntarily agree to the terms and conditions outlined in this waiver.

Cancellations and Penalties:

Cancellations must be made at least 12 hours before the start of your class to avoid losing the class credit.

If you cancel within 1 hour of the class or fail to show up, you will lose your class credit and be charged a £5 penalty.

Membership Cancellation Notice:

Notification of cancellations of memberships must be given 30 days in advance of your next payment being deducted. This must be done via email to info@thebodhiclub.com.

We may at our discretion charge an administrative fee of £30 for cancellations made within the notice period.

Health & Medical Disclosure:

I agree to inform The Bodhi Club immediately if I experience any health issues that may affect my participation in any class or activity.

I acknowledge and agree that the workouts are a recreational sports activity and may involve strenuous physical activities including, but not limited to stretches, lifts, use of props, use of reformer machines, gymnastic movements, strenuous bodyweight exercises and other strenuous activities that I am not obliged to perform, nor am I obliged to participate in any activity that I do not wish to do, and that it is my right to refuse such participation at any time during classes.

I understand that there are inherent risks in all aspects of physical exercise and I acknowledge that I have been informed of the possible strenuous nature of training. I agree that prior to my participation I will inform The Bodhi Club of any known medical conditions or factors that may place me at risk. The Bodhi Club may request a medical release from a medical practitioner prior to participation. I will inform The Bodhi Club of any symptoms before, during and after participation in a class.

Prenatal and Postnatal Clients:

I understand that certain classes or activities may not be suitable for prenatal or postnatal participants, and I have received clearance from my healthcare provider to participate.

Release of Liability:

I release The Bodhi Club and its employees, agents, contractors, and affiliates from any and all liability for any loss, damage, injury, or expense that I may suffer, or that my next of kin may suffer, as a result of my participation in any class, activity, or service provided by The Bodhi Club.

"any and all liability" — includes all negligence unless prohibited by law.

Indemnification:

I agree to indemnify and hold harmless The Bodhi Club and its employees, contractors, agents, and affiliates from any and all claims, losses, liabilities, damages, or expenses (including reasonable legal fees) arising from my participation in any of The Bodhi Club classes or activity.

Health Conditions Acknowledgment:

I certify that I have not been diagnosed with any of the following conditions; any injury, back, neck or joint pain, restricted movement, heart issues, asthma, or high or low blood pressure, arthritis, slipped or bulging vertebral disk, pelvic floor conditions, dizziness, diabetes, epilepsy, hernia, bone degeneration, high cholesterol, allergies or chronic illness. I also declare that I have notified The Bodhi Club if I am pregnant and/or have given birth in the last 12 months, or if I have undergone surgery in the past 12 months.

If any of the above health conditions apply to you, please include full details to studio staff. The Bodhi Club I shall not undertake any obligation (whether contractually, at common law or otherwise) to advise or treat me in relation to any of the matters referred to in the preceding paragraph. I acknowledge that it is my obligation and mine alone to take responsibility for my health and wellbeing during any type of exercise I undertake with The Bodhi Club.

Lost or Stolen Property:

The BODHI Club is not responsible for any lost, stolen, or damaged personal property during my time at the studio. I agree to store my belongings securely during class.

CCTV and Audio Recording Notice:

I understand that CCTV cameras and audio devices are in operation at all The Bodhi Club studios for security purposes, and that access to recorded footage is strictly controlled and limited to authorised personnel only.

Media Consent for Filming and Photography:

I consent to the use of my image, voice, and likeness in promotional materials and media for marketing and public relations purposes. I waive any right to compensation, approval, or ownership of such materials.

This applies to both photographs and video recordings.

Final Acknowledgments:

By signing this waiver, I acknowledge that I have read and fully understand the terms, conditions, and risks outlined above, and that I am voluntarily participating in the activities at The BODHI Club.
